Payment plan: $300 to start, $300 before your follow-up evaluation.

These are billed by the providers who deliver them. Your recommendation is made individually after your evaluation, so it is never sold as a pre-set package.
Paying per visit

Payment is by card through practiceQ. We don't bill health insurance: DOT SAP evaluations are an employment-related regulatory service, and insurers generally don't cover them.
