Your evaluation, explained before you book
You've been removed from safety-sensitive duty and you want to know what happens next, what it costs, and how long it takes. Here it is, without the runaround.

What to have ready
- Government-issued photo ID
- Your employer's removal or violation notice
- Your DER's name and contact details
- Any prior treatment or education records
- For telehealth: a private room and a device with a camera, in one of the seven states we serve
- Time to complete your intake forms before the visit (about 25 to 30 minutes)
Plan for about 90 minutes, somewhere you can speak freely.
$600 complete
Covers your initial evaluation, the recommendation and referrals, required documentation, the follow-up evaluation, employer reporting and Clearinghouse entries.
- Pay per visit instead: $395 initial, $250 follow-up
- Payment plan: $300 to start, $300 before your follow-up
- Education, treatment and all drug or alcohol testing are billed separately by those providers
Two things people wish they'd known sooner
Starting early costs you nothing extra. The clock on your return runs on the education or treatment recommended, not on paperwork. Every week you wait to book is a week added to the end.
You can't shop for a different answer. DOT rules don't permit seeking a second SAP's evaluation to change a recommendation, so choose a SAP you trust the first time.
